Experiential evidence shows that pain is associated with common meanings. These include a meaning of threat or danger, which is experienced as immediately distressing or unpleasant; cognitive meanings, which are focused on the long-term consequences of having chronic pain; and existential meanings such as hopelessness, which are more about the person with chronic pain than the pain itself. This interdisciplinary book - the second in the three-volume Meanings of Pain series edited by Dr Simon van Rysewyk - aims to better (...) understand pain by describing experiences of pain and the meanings these experiences hold for the people living through them. The lived experiences of pain described here involve various types of chronic pain, including spinal pain, labour pain, rheumatic pain, diabetic peripheral neuropathic pain, fibromyalgia, complex regional pain syndrome, endometriosis-associated pain, and cancer-related pain. Two chapters provide narrative descriptions of pain, recounted and interpreted by people with pain. Language is important to understanding the meaning of pain since it is the primary tool human beings use to manipulate meaning. As discussed in the book, linguistic meaning may hold clues to understanding some pain-related experiences, including the stigmatisation of people with pain, the dynamics of patient-clinician communication, and other issues, such as relationships between pain, public policy and the law, and attempts to develop a taxonomy of pain that is meaningful for patients. In this chapter we argue that the four principles of medical ethics -- beneficence, non-maleficence, respect for autonomy and justice (Beauchamp & Childress, 2001; Gillon, 1985), a new Family Interest Principle (introduced below) and a consideration of ‘capacity’ provide a reasoned practice guide for work with mothers experiencing health problems, focussing here on mental health when a parent is a patient. Our concern is the relationship of the clinician with a parent and through the parent their child. Ethics of (...) service provision or services planning (e.g. Culyer, 2001; McLachlan, 2005; see also Newbigging and Paul, chapter xxx), or the provision of other services (e.g. education, child protection) although intensely relevant to this area are not addressed in this chapter nor will we deal with the complex aspects of medical ethics relating to the treatment of children (Baines, 2008). We use the term ‘parent’ to refer to any adult person who fulfils a substantive parental role with a child. Defining what counts as a family will in certain circumstances be contentious. There are diverse patterns of family arrangements that may be influenced by cultural, political economic and temporal factors. For the purposes of our discussion, we define a family in terms of its role in childrearing, as a group of at least one adult and at least one child, living together in long term relationships on an ongoing basis with vested interest in the well being of each of the family members. Abstract:This paper is an analysis of Plato's use of the embassy to Achilles in Homer's Iliad book 9 as a literary template for Crito's mission to persuade Socrates to escape from prison in Athens. Plato's purpose is to elevate the nature of a hero by contrasting the impulsive, impetuous, mercurial temper of Achilles with the steady, thoughtful, deliberative, calmly rational argument of Socrates. Plato shows, in a volley fired at the poet, how the philosopher is more meaningfully heroic than the (...) warrior, for where the warrior aims selfishly at securing his own honor, the philosopher aims at political virtue. Despite Plato's various warnings not to do so, his dialogues have been studied as systematic philosophy since antiquity. In this innovative and controversial reassessment, James Arieti argues that they should be read primarily as works of drama rather than philosophical discourse. Analyses of 18 of the 28 dialogues allow the reader to see them as integrated dramas, with all the ambiguities and uncertainties that literary works contain. As in plays generally, the arguments of particular characters cannot be seen as (...) the opinions of the author, whose views emerge only from examining each work as a whole. This literary reading shows how much of the debate about Plato's meaning has been misplaced. Instead of demonstrating that an argument is fallacious or valid, we need to ask why Plato has the particular character make the argument. Interpreting Plato achieves what no other work on Plato has attempted: to see the philosophical arguments as serving a dramatic purpose. (shrink)

The centrality of processes of movement, translation, and transmission is already emerging in studies of topics ranging from ethnographic encounters to the history of reading. Not only does this approach offer opportunities for crossing boundaries of nation, period, and discipline that are all too easily taken for granted; it also has the potential for creating a more effective dialogue with other historians and the wider public. Since its introduction in 1987, Benjamin Freedman’s principle of clinical equipoise has enjoyed widespread uptake in bioethics discourse. Recent years, however, have witnessed a growing consensus that the principle is fundamentally flawed. One of the most vocal critics has undoubtedly been Franklin Miller. In a 2008 paper, Steven Joffe and Miller build on this critical work, offering a new conception of clinical research ethics based on science, taking what they call a “scientific orientation” toward the ethics of clinical research. Though (...) there is much to recommend Joffe and Miller’s scientifically oriented conception of clinical research ethics, I believe that both the critical and constructive projects suffer from the same basic mistake: inattention to context. The internal norms of science cannot be fully specified, let alone satisfied, independently of contextual (external) factors that only come into view when we are attentive to the particular context of that form of inquiry. Competing perspectives on the nature of emotion are illustrated with latent and emergent variable models. Latent variable models draw from classical test theory, assuming that the measured indicators of emotion covary by virtue of some common executive, organizing neural circuit or network in the brain. By contrast, emergent variable models draw from a theory-driven, operational definition tradition, positing that emotions do not cause, but rather are caused by, the measured indicators of emotion, assuming no executive neural circuit or network, and (...) requiring no covariation among indicators. We are immersed within an odorous sea of chemical currents that we parse into individual odors with complex structures. Odors have been posited as determined by the structural relation between the molecules that compose the chemical compounds and their interactions with the receptor site. But, naturally occurring smells are parsed from gaseous odor plumes. To give a comprehensive account of the nature of odors the chemosciences must account for these large distributed entities as well. We offer a focused review of (...) what is known about the perception of odor plumes for olfactory navigation and tracking, which we then connect to what is known about the role odorants play as properties of the plume in determining odor identity with respect to odor quality. We end by motivating our central claim that more research needs to be conducted on the role that odorants play within the odor plume in determining odor identity. (shrink)

Despite their crucial role in the translation of pre-clinical research into new clinical applications, phase 1 trials involving patients continue to prompt ethical debate. At the heart of the controversy is the question of whether risks of administering experimental drugs are therapeutically justified. We suggest that prior attempts to address this question have been muddled, in part because it cannot be answered adequately without first attending to the way labor is divided in managing risk in clinical trials. In what follows, (...) we approach the question of therapeutic justification for phase 1 trials from the viewpoint of five different stakeholders: the drug regulatory authority, the IRB, the clinical investigator, the referring physician, and the patient. Our analysis shows that the question of therapeutic justification actually raises multiple questions corresponding to the roles and responsibilities of the different stakeholders involved. By attending to these contextual differences, we provide more coherent guidance for the ethical negotiation of risk in phase 1 trials involving patients. We close by discussing the implications of our argument for various perennial controversies in phase 1 trial practice. (shrink)

In this book, Bradley Armour-Garb and James A. Woodbridge distinguish various species of fictionalism, locating and defending their own version of philosophical fictionalism. Addressing semantic and philosophical puzzles that arise from ordinary language, they consider such issues as the problem of non-being, plural identity claims, mental-attitude ascriptions, meaning attributions, and truth-talk. They consider 'deflationism about truth', explaining why deflationists should be fictionalists, and show how their philosophical fictionalist account of truth-talk underwrites a dissolution of the Liar Paradox and its (...) kin. They further explore the semantic notions of reference and predicate-satisfaction, showing how philosophical fictionalism can also resolve puzzles that these notions appear to present. In this paper we explain our pretense account of truth-talk and apply it in a diagnosis and treatment of the Liar Paradox. We begin by assuming that some form of deflationism is the correct approach to the topic of truth. We then briefly motivate the idea that all T-deflationists should endorse a fictionalist view of truth-talk, and, after distinguishing pretense-involving fictionalism (PIF) from error- theoretic fictionalism (ETF), explain the merits of the former over the latter. After presenting the basic framework (...) of our PIF account of truth-talk, we demonstrate a few advantages it offers over T-deflationist accounts that do not explicitly acknowledge pretense at work in the discourse. In turning to the Liar Paradox, we explain how the quasi-anaphoric functioning that our account attributes to truth-talk provides a diagnosis of the Liar Paradox (and other instances of semantic pathology) as having no content—in the sense of not specifying any of what we call M-conditions. At the same time, however, we vindicate the intuition that we can understand liar sentences, thereby avoiding one standard objection to “meaningless strategy” responses to the Liar Paradox. With this diagnosis in place, we then, by way of treatment, introduce a new predicate, ‘semantically defective’, and show how the explanation we give for its application allows for a consistent, yet revenge-immune, (dis)solution of the Liar Paradox, and semantic pathology generally.
